Disability Management

Our approach

Disability Management focuses on building a comprehensive rehabilitation plan for every client. Each plan is led by a Case Manager supported by clinical and rehabilitation staff, so services from many disciplines come together in a single care pathway.

The Case Manager’s role

• Sets rehabilitation goals together with the client and family
• Coordinates medical, therapy and vocational services
• Monitors progress and adjusts the plan to the client’s capability
• Guides the client towards independence and a return to work

Integrated medical care

Medical care is built into the disability management system, supported by doctors, nurses and therapists working as one multidisciplinary team.
